ACCESSING YOUR MEDICAL RECORDS

Dr. Gershowitz has retired from her medical practice and the office is now closed. CureMD, her electronic medical records carrier, has terminated their service - therefore individuals can no longer access and download their records through CureMD's Patient Portal.

 

You may access your healthcare records​ by requesting a copy of your chart on a CD.

REQUESTING YOUR RECORDS ON A CD

You may obtain a copy of your records on a CD for $10.00 (to cover the cost of preparation, packaging and postage). Please send a check - payable to Dr. Judith Gershowitz - to my medical records administrator at the following address:

Due to the large number of expected requests it may take several weeks to receive your CD.
